Program Assessment Checklist

The DivertGuard program assessment checklist is a structured, interactive tool that helps diversion program managers, pharmacy compliance officers, and nursing leadership evaluate whether their controlled substance program meets the standards expected during an internal audit, a Joint Commission survey, or a DEA inspection. It is available in three versions — hospital, community pharmacy, and multi-site health system — each tailored to the policies, workflows, and regulatory obligations relevant to that setting.

The checklist covers the full scope of a diversion prevention program: governance and policy, controlled substance lifecycle accountability, medication dispense cabinet controls, surveillance and analytics, physical security and camera coverage, incident investigation and reporting, and staff training. Each item carries a weight based on its relative program importance, so the real-time score reflects program strength rather than a simple item count. A score above 90% indicates a strong, well-documented program. Scores between 70% and 90% signal moderate gaps worth prioritizing. Scores below 70% suggest foundational elements are missing and warrant immediate attention.
