A licensed practical nurse who worked at a Carmel assisted living facility from December 2021 to August 2025 is accused of diverting oxycodone tablets over several months in 2025. The Indiana Attorney General's Medicaid Fraud Control Unit investigated, and two felony charges were filed: obtaining a controlled substance by fraud and failure to make, keep or furnish a record.
The discrepancy surfaced on Aug. 20, 2025, at a shift change, when an incoming nurse found that the medication cart previously used by the departing nurse contained fewer oxycodone doses than it should have. According to the affidavit, the nurse initially attributed the shortfall to an error made while feeling unwell, and coworkers reported that she appeared impaired that shift. She denied impairment but said she was extremely tired and had accidentally taken a double dose of a muscle relaxer before work.
A facility official continued reviewing records and identified 16 occasions between April and August 2025 where oxycodone was withdrawn for two residents with no documentation that the tablets were given. The facility suspended the nurse, reported the matter to state investigators on Aug. 22, 2025, and the nurse resigned. In an interview with a state drug diversion investigator, she said she must have forgotten to chart administrations on busy nights.
The nurse was arrested on June 17, 2026, and released the same day. The charges remain allegations; a trial is scheduled for Sept. 24, 2026, in Hamilton County Superior Court 4. The facility declined to comment.
